NHJFC Re-registration form - contact to get a pre-printed form. There are no blank forms. Use STJFL form otherwise.
STJFL Clearance form - for all new players from other clubs
NHJFC Proof of Age form - for all new players and any others who have not provided proof of age
NHJFC Out of Age form - for all players who are under or over the correct age for a team
NHJFC Mouthguard Request form - for all players who cannot wear a mouthguard in matches or training
